Where the data comes from
Inspection reports, compliance orders, written notifications, and monetary fines come from the Ontario Ministry of Long-Term Care public reporting. Care quality indicators, such as falls, restraints, and pain, come from Health Quality Ontario, part of Ontario Health, drawn from resident assessments. Home details such as licensed beds, ownership, and waiting-list figures come from the Ministry.
What we publish
For every active long-term care home we show the inspections we have on record, the compliance orders and written notifications inspectors issued, any monetary fines, the care quality indicators reported for the home, the waiting-list figure calculated by the Ministry, and basic facts such as beds and ownership. Each home page links to the original Ministry report behind every inspection.
How we summarize inspections
Each inspection is summarized in plain language so it is easier to follow than the official document. The summaries are generated with assistance from AI and can contain mistakes. The original Ministry report is linked on every inspection and is the authoritative version. Where there is any difference, rely on the official report.
How comparisons work
For each measure we show the home's figure beside the Ontario average for the same period, so a number can be read in context rather than on its own. Years still in progress are marked as partial. Monetary fines are shown per licensed bed so homes of different sizes can be compared fairly. Care quality indicators are reported by fiscal year, not as a multi-year average.
How issue areas are grouped
Inspection findings are grouped into issue areas based on the Ontario regulation each finding cites. This lets you see where a home was most often cited across all of its inspections. The grouping is ours; the underlying findings and citations are the Ministry's.
What we don't do
We do not assign scores or star ratings, we do not rank homes, and we do not publish top 10 or worst lists. We do not call any home unsafe. An inspection finding records a moment in time and does not, on its own, describe the quality of care a resident will receive today.
How often it updates
The site is rebuilt from the latest public data on each refresh, and each home page shows its own last-updated date. The most recent inspection record on file is dated July 6, 2026.
Corrections
If you believe something here is inaccurate, let us know and we will review it against the official record. We can correct how a record is presented; the underlying Ministry and Health Quality Ontario data can only be changed at the source.
